JOB SUMMARY Medical Social Worker I position accepts the responsibility and accountability for addressing psychosocial, financial, emotional, and environmental factors as they relate to patient health and discharge planning needs. This includes, but is not limited to, Advance Directive issues, patient/family advocacy and coordination of community resources, while maintaining professional social work standards.

ESSENTIAL FUNCTIONS
A. Conforms to the National Association of Social Workers (NASW) Code of Ethics
B. Assists patients and families with psychosocial needs including counseling as needed and coordination of family meetings
C. Recognizes problems, systematically gathers data, identifies/understands underlying issues, synthesizes complex issues, seeks input from others and makes difficult decisions to formulate appropriate treatment plans.
D. Engages and collaborates with patients, families, multi-disciplinary staff and the community in care planning, delivery and discharge. Builds solid relationships, negotiates effectively and works well with individuals of all ages, backgrounds and socioeconomic position.
E. Provides on-call services for Hospital, Physician clinics, and inpatient units.
F. Advocates for patients and helps them navigate the health care delivery system.
G. Functions as the liaison between patient, physician, adoptive agency or private attorney and patient accounts and as a support person for the mother placing her baby for adoption, both prior to and following delivery.
H. Mandatory reporter for child abuse and dependent adult abuse.
I. Helps any health care professional at MercyOne North Iowa Medical Center a possible abuse case
J. Serves as a resource person for families, patients, employees, and other health care professionals, needing information on MercyOne North Iowa Medical Center services and community agencies, programs, and services
K. Addresses and assists with development and the implementations of Advance Directives.
L. Maintains required department confidentiality and abides by HIPPA regulations.
M. Provides competent patient care to the above indicated patient populations by obtaining and appropriately interpreting data, making appropriate decisions based on demonstrated knowledge of age-specific growth and development, and appropriately adapting procedures and treatments
N. Communicates with and performs duties following the philosophy of Mercy Medical Center - North Iowa and the Sisters of Mercy reflecting the organization's Mission, Values, and Guiding Behaviors.
O. Supports and abides by all medical center, departmental, and safety policies and procedures. Proactively identifies safety concerns surrounding the work environment.
P. Performs other responsibilities as requested by supervisor, manager or department head.
POSITION QUALIFICATIONS
A. Bachelor in Social Work required, Masters in Social Work preferred
B. Experience:
a. One to three years professional social work experience preferred. Previous experience in acute care preferred.
C. Special Skills and Competencies:
a. Knowledge/skills with data systems, continuous quality improvement (CQI), community healthcare resources.
b. Ability to work independently in analyzing data, exercising judgment to arrive at conclusions.
c. Ability to establish and maintain working relationships.

f. Skill in exercising a high degree of initiative, judgment, discretion, and decision-making.
g. Ability to plan, prioritize, organize and direct the work of others.

i. Willingness to accept responsibility and to be accountable for own actions.
j. Must be able to read, write, and/or speak English.
x Must provide proof of Basic Life Support (BLS Healthcare Provider, American Heart Association; or American Red Cross, Professional Rescuer-LIVE class only) via current card prior to employment or within 60 days of hire and maintains current status.
x Completes Mandatory Reporting: Child and Dependent Adult Abuse requirement within 6 months of hire and renews this requirement every 5 years.
Completes Advanced Cardiac Life Support (ACLS) - American Heart Association within 6 months of hire and maintains current status.
Completes Pediatric Advance Life Support (PALS) - American Heart Association within 6 months of hire and maintains current status.
Completes Neonatal Resuscitation Program (NRP) - American Heart Association within 6 months of hire and maintains current status.
x Completes MANDT Training within 6 months of hire and maintains current status if assigned to Mental Health unit
Completes Trauma Nurse Core Course (TNCC) within 6 months of hire and maintains current status.
