Key Responsibilities:
- Provide direct social work case management services to participants, including participant screening, case management, referrals, and counseling.
- Participate in our Interdisciplinary Team and contribute to care planning, assessments, and re-assessments.
- Travel to participants' homes for site visits when needed.
- Participate in our quality planning process.
Requirements:
- At least 1 experience in an elderly patient setting.
- Bachelor's or Master's degree from an accredited School of Social Work with a valid social work license.
- CPR certification (we can train CPR if needed).
- Valid driver's license and valid auto insurance.
- Reliable transportation.
