JOB SUMMARY
Provides nursing services to patients and families at end of life that include assessment, planning, intervention and evaluation.

ORGANIZATIONAL SUMMARY
A. Reports to: Clinical Nurse Manager
B. Supervises: LPN, MNIU Hospice Aides, volunteers
C. Key Working Relationships: Physicians, MercyOne North Iowa Medical Center and Hospice staff
D. Primary Customer Groups: Patients, families, visitors

ESSENTIAL FUNCTIONS
A. Develops and implements patient's and individualized, holistic plan of care based on standards of nursing practice, hospice policy, and patient/family needs using clinical/critical thinking skills. Reviews and updates the plan of care based on changes in patient's condition.
B. Assesses and evaluates the patient's physical, psychosocial, environmental, spiritual, social and education needs and documents patient/family responses to nursing interventions while working collaboratively with the interdisciplinary team in the dying or comfort care process.
C. Performs all admission/discharge responsibilities.
D. Provides emotional support with end-of-life issues to patients, families, and caregivers.
E. Provides, facilitates, and/or implements learning opportunities to meet the educational needs of patient/family by assessing the learning needs and formulating and initiating a teaching plan to meet those needs with a focus on the dying patient.
F. Initiates interdisciplinary plan of care, revises care plans as indicated, and participates in IDT.
G. Effectively communicates pertinent patient changes/conditions to colleagues and to multidisciplinary caregivers.
H. Provides daily direction and guidance to Licensed Practical Nurse, Nurse Assistant, and Volunteer.
I. Accesses designated computer programs appropriately in a timely manner.
J. Maintains accurate clinical record in timely manner.
K. Collaborates with physician and pharmacy to administer medications and treatments for acute symptom management.
L. Communicates with physicians, MercyOne North Iowa Hospice staff, and peers regarding patient and family concerns.
M. Demonstrates autonomy, professional judgment, responsibility, reliability and ability to prioritize with frequent interruptions.
N. Serves as a steward for hospice human and material resources. Meets productivity expectations of unit/department as defined by Clinical Nurse Manager. Identifies/enters accurate charges. Participates in QAPI activities.
O. Collaborates with peers to complete monthly scheduling and follows staffing guidelines.
P. Demonstrates understanding of rules and regulations governing hospice services.
Q. Adapts positively to the ever-changing health care environment by supporting technology and process changes. Willing to learn new things, is open to change and maintains a calm positive manner under stressful conditions.
R. Identifies and takes action to solve problems, supports process improvement teams, which influence customer satisfaction, clinical outcomes and financial stability.
S. Documents patient information in the electronic/paper medical record in an accurate, timely manner.
T. Delegates/supervises appropriately based on patient needs and the scope of practice.
U. Maintains required department confidentiality and abides by HIPPA regulations.
V. Provides competent patient care to the above indicated patient populations by obtaining and appropriately interpreting data, making appropriate decisions based on demonstrated knowledge of age-specific growth and development, and appropriately adapting procedures and treatments.
W. Takes an active role in enhancing ability to carry out job functions through personal and professional/job-related growth and development and participates in medical center and/or departmental education programs.
X. Communicates with and performs duties following the philosophy of MercyOne North Iowa Medical Center and the Sisters of Mercy reflecting the organization's Mission, Values, and Key Results.
Y. Supports and abides by all medical center, departmental, and safety policies and procedures. Proactively identifies safety concerns surrounding the work environment.
Z. Performs other responsibilities as requested by supervisor, manager or department head.

POSITION QUALIFICATIONS
A. Education [formal]:
1. Current RN licensure in the state of Iowa or qualified multi-state license required
2. Associate Degree in Nursing or graduate from an accredited RN program required.
3. Bachelor of Science in Nursing preferred.
4. Hospice and Palliative Care Certification preferred
B. Experience:
1. Must have demonstrated nursing competence in an acute/long-term care patient care setting
2. Caring for terminally ill patients/families, Hospice or home care nursing experience is preferred
C. Special Skills and Competencies:
1. Demonstrates empathy and belief in the hospice philosophy
2. Demonstrates ability to work independently, make autonomous decisions, yet function within a team structure
3. Demonstrates good organizational skills
4. Demonstrates a commitment to individual development by attending meetings and in-services
5. Obtain Port Access Certification within one year of hire date
6. Completes the following mandatory courses (check all that apply)

X Must provide proof of Basic Life Support (BLS Healthcare Provider, American Heart Association; or American Red Cross, Professional Rescuer-LIVE class only) via current card prior to employment or within 60 days of hire and maintains current status.

X Completes Mandatory Reporting: Child and Dependent Adult Abuse requirement within 6 months of hire and renews this requirement every 3 years.
Completes Advanced Cardiac Life Support (ACLS) - American Heart Association within 6 months of hire and maintains current status.
Completes Pediatric Advance Life Support (PALS) - American Heart Association within 6 months of hire and maintains current status.
Completes Neonatal Resuscitation Program (NRP) - American Heart Association within 6 months of hire and maintains current status.
Completes MANDT Training within 6 months of hire and maintains current status.
Completes Trauma Nurse Core Course (TNCC) within 6 months of hire and maintains current status.
