PeaceHealth is seeking a RN Minimum Data Set Coordinator - Long Term Care for a Full Time, 1.00 FTE, Day position. The salary range for this job opening at PeaceHealth is $44.72 - $67.08. The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc.

Accountable for coordinating and overseeing the full collaborative, interdisciplinary assessment and care planning process in this unit. Promotes the resident's quality of care and services through assessment and analysis of quality indicators and development of performance improvements. Assures appropriate reimbursement through accurate and timely compliance with federal/state requirements.

Essential Functions

  • Plans, organizes develops and coordinates the completion of documentation and the input of patient assessment data as required by federal and/or state regulations for reimbursement. Assures accurate transmission of data in accordance with regulations and maintains a compliance log of transmissions.
  • Leads the development, implementation, outcome and evaluation of a plan of care for each resident in the unit. Leads interdisciplinary teams to ensure resident issues/needs are identified and care plan and goals are established and measured and adapted to ensure goals are met.
  • Ensures accuracy of care plan and endorses and certifies by their signature on the care plan.
  • Maintains updates and revises required documentation according to patient changes in condition, as regulated by federal and/or state guidelines. Effectively communicates and coordinates with members of the interdisciplinary team regarding appropriate functional/care level and transition to another level. Advises patients of rights regarding information transmitted.
  • Performs audits related to the reimbursement documentation and the quality management process. Reviews audit results and presents summary information and recommendations for management utilization and review. •Develop, implement, and maintain an ongoing quality assurance program for the resident assessment/care plans.
  • Ensures compliance from a risk management/regulatory perspective through early identification of clinical events or trends. Provides data management and summary information regarding clinical decisions for benchmarking and comparative analysis.
  • Maintains education and updated knowledge of federal and state regulations in order to assure appropriate reimbursement. Provides education and guidance to the interdisciplinary team regarding regulations, reimbursement system, and associated documentation to assure maximum reimbursement levels.
  • Performs all functions according to established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards.
  • Maintain and update written policies and procedures that govern the development, use, and implementation, of the resident assessment (MDS) and care plan, in collaboration with the unit Director and/or Charge Nurse.
  • Performs other duties as assigned.
Qualifications

Education
  • Bachelor's Degree Preferred.
Experience
  • Minimum of 3 years Required: Clinical experience
Credentials
  • Required: Registered Nurse and
  • Required within 2 years: Resident Assessment Coordinator - Certified
Skills
  • Must have good communication skills. (Required)
  • Must have strong organization skills and be able to work independently. (Required)
  • Knowledge of quality improvement processes with an emphasis on clinical documentation standards, auditing, and reimbursement process. (Preferred)
Working Conditions

Lifting
  • Patient handling no greater than 35 lbs. without the use of assistive equipment and/or devices (NIOSH).
  • Fine motor skills to be able to grasp and control medical equipment and perform precise procedures.
  • Push/Pull: Frequently up to 45 lbs. force (i.e., WOW, medical carts).
  • Perform hands on CPR at least 20 mins using 100-125 lbs. of force (National Assoc. of EMS Physicians and AHA) Frequency should be seldom or occasionally.
  • Ability to move around area with frequent sitting.
  • Bending/ stooping/ squatting/ reaching/ kneeling frequently.
  • Lifting (non-patient) up to 20 lbs. occasionally and 5 lbs. frequently.
Environmental Conditions
  • Exposure to biohazard, body fluids and airborne particles.
  • Must be able to complete tasks in a noisy environment.
Mental/Visual
  • Vision and hearing required within normal limits (glasses, contacts, hearing aids permitted).
  • Ability to communicate and exchange accurate information.
