Your role is crucial in achieving high member satisfaction within the Goodwin Living at Home program. Key Duties: Network Development:

Collaborate with the Manager of Member Experience to build and maintain a network of top-tier service providers, including home care agencies and health care providers. Member Assessments:

Conduct pre-admission, annual, and as-needed assessments to determine the specific needs of each member, ensuring they receive appropriate services. Member Engagement:

Regularly communicate with members through visits, phone calls, and meetings to ensure their satisfaction and address any concerns. Service Coordination:

Manage the placement and quality monitoring of services, ensuring members understand their coverage and financial responsibilities. Community Outreach:

Participate in informational seminars and networking activities to enhance community awareness of our continuing care at home program. Job Requirements: Educational Background:

Bachelor’s degree required; a degree in social work is preferred. Experience:

Experience with older adults and providing in-home services is a plus. Skills:

Strong communication skills, both verbal and written. Proficiency with Windows operating systems, MS Office, and email software. Attributes:

Ability to work independently, manage time effectively, and exhibit a high degree of initiative and creativity. Commitment to quality care and sensitivity to the needs of older adults. Flexibility:

Must be able to work flexible hours, including rotating after-hours on-call duties, and handle members’ needs during nights and weekends as required.
