Who we are: Leo A. Hoffmann Center is a nonprofit agency that provides Psychiatric Residential Treatment Facility (PRTF) and outpatient mental health services. Our outpatient mental health services, which include CTSS and ARMHS Services, reach individuals of all ages and demographics in the community setting in our local communities of St. Peter, Mankato, New Ulm, and Waseca. Our PRTF services provide treatment to adolescent males with complex mental health conditions at our St. Peter campus. This is an intense and collaborative environment that specializes in care that is more intensive than traditional residential services and serves individuals who do not require psychiatric hospitalization. What we're looking for: This position is responsible for coordinating and facilitating referral, admission, and discharge of residential services. A professional in this role serves as the point person of communication for referral agents and any discharge referral or planning coordination. This professional is efficient and effective, ensuring a positive experience for clients, families, and referral sources during the transitional times of admission and discharge from residential programming.
