$35,000.00 To 54,706.00 Annually Hours: full-time (37.5 hours per week), Monday through Friday during school hours Location: Lynchburg, VA 24501 POSITION SUMMARY The Care Coordinator will serve in the school system and will engage with all students attending both domestically and enrolled with Horizon BH services, to meet the mental health needs within the school system. The Care Coordinator will engage, conduct initial screening, provide supportive level counseling and/or education, and refer students to a higher level of care. This position will be assigned to a public school location Monday through Friday. REQUIRED EDUCATION AND EXPERIENCE Bachelor's degree in psychology, social work, counseling or human services field One (1) year of relevant clinical experience QMHP-C RELATED KNOWLEDGE SKILLS AND ABILITIES Knowledge of psychotherapeutic interventions with education and mental health or substance abuse treatment; Demonstrated understanding of youth mental health presentation and services available for youth with presentation of mental health concerns; Ability to engage with youth, school administration officials, and teachers in professional manner; Ability to articulate screening and assessment outcomes to parents, students, and administrators, and articulate next plan for client care; Ability to make referrals and linking to appropriate behavioral, medical, social services; Ability to develop and maintain therapeutic relationships; Ability to model appropriate behavior and teach the use of daily living skills to enhance social and interpersonal skills; Ability to respond appropriately to aggressive behaviors with least restrictive interventions; Strong written and spoken communication skills; Ability to work in and contribute to a collaborative team environment and promote positive, collaborative, and professional working relations among co-workers and representatives in the community; Ability to record data and complete documentation within established expectations and by communicated deadlines. Willing and available to work modified schedules (weekends, holidays, etc.) as required; Ability to drive and must possess a valid driver’s license. May be called upon to travel to any Horizon worksite; transport clients, provide services to clients in other locations; Ability to participate in and successfully pass behavioral intervention training; includes both verbal de-escalation skills and physical skills techniques to be utilized with consumers as needed.
