Transportation & Scheduling Specialist

The LIFE PACE (Program of All-Inclusive Care for the Elderly) Clinic team performs a vital service by providing primary care for Tulsa's aging population. The goal of our amazing team of professionals is to provide outstanding services and oversight in order to keep participants in their homes for as long as safely possible in lieu of nursing facility placement.

We are looking for a Transportation & Scheduling Specialist who will join a team of individuals dedicated to providing unique health care for Seniors The Program of All-Inclusive Care for the Elderly (PACE) offers older adults and their caregivers a welcome alternative to nursing home care. Participants have access to comprehensive medical services and social support at the site while they enjoy independent living in their own homes and communities.

The Transportation & Scheduling Specialist is responsible for scheduling transportation for the participants in the PACE program. This includes transportation to and from their home to our medical clinic, outside doctor's appointments, medical testing facilities, Adult Day Health and other destinations as determined by the participants care plan. In addition, troubleshoots service delays, passenger logistics issues and appropriate reporting of service utilization and cancellations.

•Understands and can articulate the LIFE Transportation department philosophy.

•Possesses general knowledge of all agency programs, policies and procedures.

•Schedules transportation for outside appointments, ADH and other participants needs as required.

•Reviews transportation schedule daily for accuracy.

•Submits the next day's manifest by 2pm each day to the transportation providers.

•Reports on service utilization

•Participates as a member of the PACE IDT

•Participates in POC

•Notifies participants of scheduled appointments and sends reminders by mail, phone, email, etc.

•Troubleshoots service delays and passenger logistics

•Reports immediately on service cancellations and documents "no show" and cancellations for all appointments.

•Performs additional duties as required or assigned.

•Learn driver responsibilities and occasionally transport clients when needed.

•On-Call is required.

•Possesses general knowledge of all agency programs, policies and procedures.

•Performs additional duties as required or assigned.

•Communicates effectively with external vendors, Care Managers and PACE staff

•Handles confidential information appropriately and in compliance with all HIPAA regulations.

Requirements

EDUCATION AND EXPERIENCE:

•High school diploma or GED required.

•Current driver's license with good driving record.

•Prefer one year of experience in a busy office environment, preferably in health care of transportation.

KNOWLEDGE, SKILLS, and ABILITIES:

•Excellent verbal and written communication skills.

•Exceptional interpersonal skills to facilitate work relationships.

•Advanced knowledge of computer technology & Microsoft Office a must.

•Positive, professional attitude & appearance.

•Ability to understand written and verbal instructions.

•Ability to effectively present information and respond to questions on the telephone and in person, from groups of managers, clients, customers, and the public.
