Job Description

Position Title: PRS Case Manager Level 2

Reporting Relationship: PRS Caseworker Supervisor

Position Summary

Working under a grant from the Office of Refugee Resettlement (ORR), the PRS Case Manager Level 2 provides level 2 post release services including conducting initial in-home assessments, ongoing monthly assessments, plans, connection to community-based services, and other services either in-person or virtually, depending on wellbeing of the child. PRS Case Managers must have knowledge of local community social services and specialized experience working with immigrant populations and should be knowledgeable about family preservation, kinship care, and/or general child welfare. PRS case managers provide Level Two (2) PRS and may also provide Level One (1) PRS. Required travel includes initial in-home assessment followed by monthly in-home or virtual visits with in-person home visits at least every 90 days.

RESPONSIBILITIES
  • Perform all work in a manner consistent with the National Youth Advocate Program's Mission, Values and Philosophies.
  • Post Release Case Managers provide Level Two (2) post release services. They may also provide Level One (1) post release services as necessary.
  • Conduct an initial in-home assessment. If there are no safety or wellbeing concerns, case managers may conduct subsequent monthly engagements either in-person or virtually but must complete an in-person home visit at least every 90 days.
  • Conduct ongoing assessments, make safety plans, create strengths-based plans with children and their sponsors, and refer and connect children to community-based services.
  • Carry a minimum caseload of 15, if regionally based, or 8, if cases require travel outside the area.
  • Ensure excellence in PRS documentation and Program compliance.
  • Actively assist in establishing working relationships with community-based organizations responsible for social services in their proposed area of operations.
  • Provide and foster a coordinated case management approach that adopts a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet the unaccompanied child and the sponsor's presenting needs.
  • Ensure proper reporting and elevation to ORR of timely notifications of concern and reporting to other stakeholders per state and ORR guidelines.
  • Provide and ensure effective communication with community partners, community-based organizations, and ORR stakeholders.
  • Demonstrate working knowledge and understanding of ORR Policy and Procedures, Flores Agreement as well as other applicable, and pertinent regulations and laws including the Trafficking Victims Protection Reauthorization Act (TVPRA) of 2008 et.al.
  • Ensure protection of Personal Identifiable Information (PII) and adhering to confidentiality of information and HIPAA regulations per ORR Policy requirements.
  • Establish a respectful, supportive workplace environment that promotes NYAP's values of caring for people, connecting communities and promoting peace while ensuring staff accountability and compliance.
  • Elevates any issues or concerns to NYAP leadership and ORR.
  • Perform other duties as assigned.
MINIMUM QUALIFICATIONS
  • Applicants must be at least 21 years old.
  • Bachelor's degree in social work (BSW) or an equivalent degree in education, psychology, sociology, or other behavioral science equivalent.
  • Knowledge of local community social services and should.
  • Specialized experience working with immigrant populations.
  • Knowledgeable about family preservation, kinship care, and/or general child welfare.
  • PRS case managers must be bilingual in English and Spanish or have access to professional interpretation services.
  • Prior experience with ORR HS/PRS preferred.
  • Proficient in the use of desktop and laptop computers, using the internet and related computer programs including ORR's Official System of Record (also known as the UC Portal).
  • Pass a criminal history screen, including state and local child protection agency registries, and C/ANs check.
  • Ability to travel up to 75% of the time.
  • Active motor vehicle license, reliable automobile, a good driving record, and 100/300 insurance coverage on personally owned vehicle.
  • Willingness to work flexible and non-traditional hours in the service of persons served.
