The Intake Specialist establishes records for new persons served. Additionally, this position files clinical documentation and monitors records on a regular basis to ensure that required documentation is submitted in a timely manner consistent with applicable standards.

Job duties include but are not limited to:
    • Identifies strengths, treatment needs, and any supports (natural or otherwise) of persons served
    • Determines needs of persons served by an assessment of areas of daily living and functioning to include: mental health, physical health, substance abuse, employment, housing, and skills in areas such as socialization, problem solving, coping, money management, and time management
    • Completes intake documentation within timelines determined by Medicaid and DMH, and any other relevant funding or accrediting bodies
    • Creates collaborative treatment plan with persons served in an effort to identify goals they wish to achieve while working with PFP
    • Informs persons served of expectations regarding services, focus on services and supports PFP can offer in achieving goals
    • Attends weekly team meetings to promote and facilitate team coordination and continuity of care
    • Attends weekly supervision to review eligibility decisions and service determinations on new persons served
    • Assists persons served to reinvigorate a healthy system of emotional and social supports; provide a systemic intervention of helping families and others who may have once provided significant support
    • Coordinates successful transition of care from Intake to CPR/ACT teams for persons served
    • Coordinates correspondence with other agencies to obtain and provide client clinical records as needed
    • Maintains quality assurance records and files according to agency certification and accreditation requirements
    • Monitors clinical documentation of CPR contractors to assure completeness and timeliness of their CPR client records
    • Completes annual training requirements as outlined by agency requirements and supervisor
    • Performs other related tasks as assigned by the supervisor
Requirements
  • Master's Degree in a social services field
  • Working knowledge of the DSM, psychotropic medications, and substance use disorders
  • Experience with Electronic Medical Records and working knowledge of Microsoft Word and Excel.
  • CPR and First Aid certifications
