Harbor is looking for a Manager of Clinical Transition and Multi-System Youth who provides clinical leadership, management and consultation for Care Coordination services within assigned catchment area(s). Provides operational oversight for MSY (Multisystem Youth Funding) applications, PRTF (Psychiatric Residential Treatment Facility) prior authorizations, and Flex Fund utilization for the CME Department.

Position is full-time, and candidate preferably lives in one of the 11 counties we work with: Wood, Hancock, Seneca, Crawford, Wyandot, Marion, Erie, Sandusky, Wyandot, Huron, or Union.

Education/Experience/Other Requirements:

  • Master's degree in a mental health or related field required with experience in Management or Clinical Supervision.
  • State of Ohio Clinical licensure (LISW, LPCC, LISW-S or LPCC-S, RN) or qualifications as determined appropriate for the position.
  • At least 5 years of experience in program administration in behavioral health, program design and implementation, providing services to youth and their families or caregivers, reporting and data management, and compliance.
  • Must have experience in at least one of the following areas of expertise: family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development.
  • Culturally competent or responsive with training and experience necessary to manage complex cases
  • Qualifications and experience to work with children and families experiencing SED, trauma, co-occurring behavioral health disorders and engaged with one or more child-service systems.
  • Excellent leadership and communication skills required.
  • Must be a team player, honest, self-motivated and highly organized.
  • Prior Wraparound or Intensive Home-Based Treatment experience preferred.
  • Prior clinical/behavioral health and administrative management experience.
  • Prior experience in grant writing preferred.
  • Able to effectively communicate and supervise care coordination services to motivate staff to achieve maximum results.
  • Work with Quality Improvement department to ensure management and compliance of department training.
  • Demonstrated ability in developing and maintaining collaborative community relationships.
  • Able to work successfully with interdisciplinary teams.
  • Must have valid driver's license, acceptable driving record, and be deemed insurable under Harbor's vehicle insurance carrier.
  • Must be proficient and accurate in computer use, including an electronic health record system and Microsoft Office 360.
  • Required to complete High Fidelity Wraparound training and successfully complete skill and competency-based training to supervise delivery of ICC and MCC.
  • CPR/First Aid and NCI/CPI Certifications required within 90 days of employment.
Essential Job Competencies/Primary Duties:

  1. Exhibits leadership and team-building skills to promote a positive culture among the team, within the department, and across the agency.
  2. Outstanding organizational skills with high attention to detail and accuracy.
  3. Demonstrates leadership and supervision skills that lead to expected levels of productivity, efficiency and effectiveness of team members in expecting and providing excellent care to clients.
  4. Responsible for management of MSY, PRTF, and Flex Funding for the department
  5. Reviews and analyzes feedback from youth and family/caregivers served, Child Family Team members, and program measures and provide CME Director with recommendations for programmatic changes to improve as necessary.
  6. Provides personnel management functions for all assigned staff including supervision, involvement in hiring, disciplinary actions, etc. Any disciplinary action involving time off without pay or termination requires coordination/consultation with Human Resources.
  7. Actively participates in community meetings and workgroups focused on enhancing services and community capacity to effectively serve youth and families.
  8. Assists in innovation, development and implementation and evaluation of services, including outcome and grant reporting
  9. Provides care coordination services as needed.
  10. Provides leadership and supervision to support the team in fulfilling or exceeding program contractual requirements.
