for a team of MDS professionals and requires an in-depth knowledge of geriatric nursing and medical knowledge. You will be responsible for coordinating and managing the Minimum Data Set (MDS) process and ensuring compliance with regulatory standards. You will play a crucial role in assessing patient care needs, developing care plans, and ensuring accurate documentation. The winning candidate will have a minimum of two (2) years experience in geriatric and MDS/PDPM setting. Excellent communication skills, including the ability to communicate with the IDCP team and staff nurses.

Responsibilities: - Coordinate and manage the MDS process, including assessments, care planning, and documentation - Collaborate with interdisciplinary team members to develop comprehensive care plans - Ensure compliance with federal, state, and local regulations regarding MDS assessments and documentation - Conduct thorough assessments of patients' medical conditions, functional abilities, and psychosocial needs - Utilize medical management principles to develop individualized care plans - Monitor and evaluate the effectiveness of care plans and make necessary adjustments - Stay informed about changes in medical coding standards (such as ICD-10) and implement necessary updates - Provide education and training to staff members on MDS processes and documentation standards

Skills: - Strong knowledge of patient care principles and practices in an acute care setting - Proficiency in medical terminology and physiology - Experience with managed care standards and regulations - Excellent communication skills, both written and verbal - Attention to detail and accuracy in documentation - Ability to work collaboratively as part of an interdisciplinary team - Strong organizational skills with the ability to prioritize tasks effectively
