OVERVIEW: The Intake and Assessment Specialist will screen at-risk individuals and families. The screening tool used will be the HPAT. This position will be familiar with community referral resources. This position will work closely with the Housing Coordinator to process rental applications and the Case Managers to ensure clients are connected to needed resources and support to achieve housing and financial stability. The Intake and Assessment Specialist will report directly to the Director of Social Services.DUTIES AND RESPONSIBILITIES:Manage the daily flow of clients needing to be screened for rental assistanceScreen at-risk individuals and families using the HPAT ToolProvide referrals to necessary services for clientsProvide internal referrals to connect clients with food, meals, bus passes, and seasonal programs.Establish and maintain relationships with other community agenciesMaintain accurate client records in Wellsky, The Salvation Army client software, HMIS system, and hard filesParticipate in a team effort to meet the needs of the clients served by the department.Attend all required meetings and trainingsAssist in seasonal programs, bus pass sales, covering the receptionist, and food pantry registrationOther related duties as requiredEDUCATION AND EXPERIENCE: High school diploma or equivalent (required)Bachelor's degree in Social Work or related (preferred)Experience with Microsoft programsExperience with the Homeless Management Information System (HMIS) databaseExperience working with persons from diverse ethnic and cultural backgrounds who are homeless and economically disadvantagedSKILLS AND ABILITIES:Bilingual in Spanish (preferred)Excellent communication skills both written and verbalAbility to leadExcellent time management and organizational skillsProblem-solving skillsAbility to multi-taskTeam playerPHYSICAL REQUIREMENTS:Ability to lift up to 25 lbs.
