Esse Health is currently seeking a full-time Care Manager - Nurse Practitioner.

This role is focused on home care for Medicare patients that need facility post-discharge follow-up (hospital, SNF, urgent care).

The Nurse Practitioner (NP) is an advanced practice Registered Nurse that provides service through core competencies of direct care, consultation, expert guidance, leadership, ethical decision making and collaboration. The NP Care Manager is a member of the care management team and works under the supervision and direction of a physician to improve the health of patients. The NP will use their advanced clinical skills and training to assess, diagnose, manage and treat. Essential to the role of the NP care manager is the ability to collaborate and participate in care coordination activities that meet the patient needs. These skills are essential to reducing barriers when managing discharge planning, directing transitions of care, and assisting patients throughout their health care journey.

Qualifications:

  • Education: Master's Degree of Science in Nursing.
  • Licensure/Certification: Licensed and certified as an Advanced Practice Nurse in Missouri and Illinois
  • Years of Experience: 2 or more years' experience as an Adult, Family or Gerontology Nurse Practitioner
