**Medical Records Technician**

**Position Summary**: Primarily responsible for chart preparation, medical records requests and scanning of all incoming faxes as well as day-to-day administrative and clerical functions as they relate to the operation of medical records. A Member of the support staff team.

**Supervisor**: Medical Records Lead Coordinator

**PRIMARY RESPONSIBILITIES**:

- **Prepare patient charts in anticipation of appointment**:

- **Retrieve patient medical records for physicians, technicians, and other medical personnel**:

- **Protect the security of medical records to ensure that confidentiality is maintained**:

- **Process medical record releases and release information to persons or agencies according to regulations
**- **Process medical records in coordination with other providers**:

- **Receive and scan medical records into electronic medical records**:

- **Maintain database of all daily office notes**:

- **Fax daily completed office notes to referring providers**:

- **PFT prep and faxing to PCP & Referring Provider**:

- **Phones to handle record requests from patients to coordination of care from other practices.**:

- **Completing Billable request for records.**:

- **Chart Audits**:

- **Other duties as assigned

**GENERAL**:

- Maintains strict confidentiality.
- Presents a polite and courteous manner to the patients at all times, and be willing to meet the patient’s needs.
- Participate in compliance of OSHA/HIPPA/OIG regulations.
- Other duties as assigned.
- Normal hours-see schedule, one-half hour lunch

**JOB REQUIREMENTS**:

- High school education or G.E.D. equivalent.
- Previous medical experience in an outpatient setting with electronic medical records experience preferred
- Understanding of medical terminology. Completion of course for a Medical Assistant would be an advantage.
- Proficient computer skills with database experience
- Strong organizational skills with attention to detail
- Reliable, motivated, team player
- Ability to multi-task and prioritize work
- Ability to read written documents
- Professional written and verbal communication skills

**WORKING ENVIRONMENT**:

1. **Physical Demands**:
Lifting Requirements: 10-20 lbs.

Frequency of Lifting: 0-25% of the time

Average percent of time during regular shift devoted to:

Hearing: Adequate to perform the essential functions of the job such as answering the telephone and speaking with colleagues.

Speaking: Adequate to perform the essential functions of the job such as communicating via telephone and in person for the purposes of appointment scheduling and providing patient information.

Dexterity: Adequate to perform the essential functions of the job such as sorting papers; stapling; operating computer keyboard or typewriter.

Mental Demands: Adequate to perform the essential functions of the job such as calculating and organizing files. Position may periodically encounter unfriendly or hostile individuals regarding requests, conflicts, etc.

3. **Working Conditions**:
Position requires work in normal office environment 100% of the time. No special uniform or protective equipment is required.

Hazardous materials exposure mínimal and limited to normal office supplies.
