The BEST person for the job is the one that is willing to work as a team member, drawing from the resources available, to exceed the Patient's and the Patient's family's expectations. Working with a team of professionals including Nurses, Doctor, Nurse Practitioner, LVN, C.N.A. ,Social Worker, and Spiritual Coordinators will ensure the BEST hospice care is always provided.
The BEST RN-Case Manager selected can expect:
- To receive support from our office staff and medical professionals as you go about meeting the needs of our patients.
- To be assigned a case load of patients that you will coordinate care for with support from other Team Members
- Able to take call after hours on average of 3 days per month
- To document in electronic medical records all services you provided for our patients and finalize documentation timely.
- To maintain supplies, medications and equipment by ordering items in anticipation of our patient's needs.
- To participate, support, encourage and take pride in the Team that is Best Hospice Care of Texas
Pay: $72,800.00 - $81,120.00 per year
