_**Certified Family Peer Advocate / Case Manager**_

**Duties & Responsibilities**:

- Sharing personal account of your child with special needs and how it impacted your daily life, family cohesion, and services that were and were not beneficial
- Case management duties:

- Assist in the assessment of family needs
- Assist in the development of family-driven, strength-based service plans
- Collaborate with other systems (case managers, therapists, school personnel, hospitals, etc.) to ensure access and quality of care is consistent with client need
- Skill building duties:

- Provide various skill building services to clients
- Provide mentoring to identified clients with both personal and first-hand experience with system navigation, evidence-based behavioral intervention strategies, and crisis management protocol
- Administrative duties:

- Meet with the Supervisor of the Community and Home Based Services for weekly supervision
- Attend weekly group supervision with entire Community and Home Based Services team
- Keep all required paperwork up to date
- Perform other duties consistent with job classification, as assigned by supervisor
- Lived experience navigating services for a child with special needs
- Bachelor Degree in Human Service field
- General understanding of the emotional, social, cultural, financial, educational, health, and legal issues families face
- Basic knowledge of community resources
- Ability to develop rapport with troubled children, youth, and their families
- Valid driver’s license

Pay: $42,000.00 - $50,000.00 per year
