**SUMMARY**: As part of the SLS Management Team, the Community Support Facilitator (CSF) is responsible for overseeing the scheduling and support of Direct Service Support staff. Under the direction of the Program Director, the CSF establishes and maintains good client relations and service by directing, coordinating and managing staff. Ensures coverage of shifts and serves as a back-up for staff temporarily absent.

**REPORTS TO**: Program Director

**ESSENTIAL DUTIES**:Include the following (other duties may be assigned):

- Report directly to the Program Director
- Supervise, train, and provide guidance to the direct client support staff in accordance with state, federal and funding authority regulations, Lighthouse Living Services personnel policies, the Supported Living policy, and procedure manual
- Participate as part of an interdisciplinary team in the development of each client’s Individual Support Plan (ISP) and Person-Centered Plan (PCP); assess effectiveness of achieving objectives
- Assist with direct service for each client
- Actively help clients find permanent housing
- Complete daily, weekly, monthly, and annual documentation accurately, and as required by Lighthouse and state and federal regulations; ensure timely submission
- Maintain on site client files according to standards and protocol established.
- Facilitate all repairs and required maintenance of a client’s home
- Responsible for following client’s budget and ensuring payment of bills
- Assist client in maintaining Medicaid eligibility by scheduling and attending all Medicaid appointments and providing proper documentation
- Communicate professionally with families, Case Managers, Behavioral Consultants, health care personnel, schools, landlords, employers, etc.
- Implement training programs as specified in each client’s ISP
- Comply with behavior support policies and procedures and implement approved behavior support programs
- Ensure proper medication/treatment administration and assist in meeting the clients’ health care needs
- Schedule and assist clients in all medical appointments and procedures. Ensure completion of proper follow up.
- Plan and implement community-based, age-appropriate, recreational activities for the clients
- Respond appropriately and promptly to on-call situations.
- Report any suspected abuse, neglect, or exploitation to Program Director and proper authorities
- Participate in all meetings as needed
- Oversee Special Incident Reporting and follow up with Program Director to relay details
- Assist the Program Director in various projects
- Organize the timely and accurate submission of mileage logs
- Check IHSS timesheets
- Other duties as assigned by Program Director

The duties described above are not inclusive. Additional duties may be required from time to time due to the need to respond to clients and to remain competitive in the marketplace.

**PHYSICAL DEMANDS**: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. The Program Supervisor is frequently required to walk, sit, stand, drive and use hands to type, write reports, file paperwork, etc. Specific tasks required include writing, using a computer, calculator, telephone, etc. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of the position.

**WORK ENVIRONMENT**: The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job.

The normal environment is an office, which is environmentally-controlled. Additionally, while performing the duties of this position, the employee may be required to work, walk and drive in a variety of seasonal weather conditions associated with the locality and/or the Program. May also be required to work in consumers’ residences, which have varying conditions, including those that might not be hygienic or sanitary.

**EDUCATION AND/OR EXPERIENCE**:

- One year of experience in a human services delivery system, preferably in a comparable program, or
- Completed at least one year of post-high school education in a human services-related field or specialization
- Demonstrated ability to provide staff training and planning
- Minimum of 6 months of supervisory experience
- Minimum of 6 months of professional experience in the program to be supervised
- At least 18 months’ experience and/or training on working with clients with behavioral issues
