**Position Summary**:Arranges or partners with other health professionals regarding health and safety standards for out-of-home childcare, childcare licensing requirements, disease reporting requirements for care providers, immunizations for children, injury prevention for children, medication management and knowledge of community health and mental health resources for children.

**Essential Duties and Responsibilities**:To perform this job successfully, an individual must be able to perform the following satisfactorily; other duties may be assigned. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
- Coordinate team based medical care for the children placed in the center.
- Prepare, compile, submit, and maintain accurate medical records, files, forms, statistics, and additional relevant information in accord with agency policy, licensing and/or funding requirements.
- Review ORR portal for compliance with medical information upload requirements and ensure updates are input as needed for safety and timely discharge.
- Complete calls with sponsors as assigned to provide medical information.
- Actively participate in medical and service-related resource development prior to UC discharge.
- Participate in planning, coordinating, implementing, and evaluating all medical needs of the UC for designated social service program(s).
- Provide documented information related to identified delays in the medical assessments.
- Attend scheduled meetings and participate in team decisions and operations to represent the medical needs of the UC.
- Coordinate medical resources for all employees and resource family homes.
- Adhere to Child Abuse and reporting requirements.
- Must be relied upon regarding task completion and meeting deadlines without sacrificing accuracy, work quality, or customer service satisfaction.
- Attending meetings and training when needed or requested.
- Perform other duties as requested by the assigned supervisor and/or management.

**Supervisory Responsibilities**: Manages subordinate employees. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws.

**Qualifications**:To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. - 2+ years case aid experience in progressively challenges.
- 2+ years working experience with children and families preferred.

**Education/Experience**: Associate's degree or equivalent from two-year college or technical school; or six months to one year related experience and/or training; or equivalent combination of education and experience.

**Associate degree preferred. RN, LVN, BSN license. Two to four years’ experience working with children and families.**

**Language Ability**:
**Bilingual in Spanish required.**
