Position: Case Manager Youth Residential Program

Status: Full-Time Introductory Period: 90 days

Reports to: RHY Director

Purpose:
The Case Manager is responsible to create an environment of trust and compassion that allows our youth ages 12-17 to grow and develop. The Case Manager will fully assess the diverse needs of all the youth assigned, provide psycho-social support and life skills. The Case Manager will also provide the youth and/or family with needed resources. The Case Manager will ensure that all individual treatment plan needs are being met and all services are provided in accordance with the Youth Emerging Stronger mission statement, Value Statement, Code of Conduct and YES Policies and Procedures.

Responsibilities and Duties:

- Cultivate an environment of caring that offers the youth a chance to develop positive relationships and connections with adults and ensure quality care to residents in the program.
- Provide strengths-based case management and service coordination services designed to assist clients with family reunification and/or appropriate permanency placement
- Coordinate with other organizational entities such as DCFS, DMH, Probation, surrounding School Districts, Homeless Youth Liaison, Youth Coordinated Entry System (CES), etc.
- Maintain an attitude of professionalism, collegiality, and optimism with both staff and youth to promote the welfare and well-being of the youth.
- Oversee and conduct timely comprehensive intakes (within 24 business hours) to include the needs & services plans of each youth.
- Use all available data to create an individualized service plan that identifies youth’s strengths and challenges; help youth to plan short-term and long-term goals.
- Update Contact Roster and HMIS weekly
- Cultivate a working knowledge and relationship with relevant community resources to create linkages that meet the needs of the youth and/or youth’s family.
- Maintain accurate and timely documentation that meets agency and legal requirements; complete monthly and weekly reports.
- Periodically transport and accompany youth to meetings and appointments with entities such as DCFS, medical providers, court, and mental health providers etc.
- Support and promote positive teamwork amongst employed staff at YES.
- Maintain professional relationships with other community providers and families of youth in the program.
- Assist in the training and guidance of onboarding new staff and interns on agency policies and procedures.
- Attend all staff meetings, in-service trainings, supervisions and conferences as directed.
- Ensure that all services provided are meeting agency and national accreditation standards.
- Ensure parent/legal guardian is contacted with in 72 hours of youth arriving to program to obtain consent for youth program participation.
- Ensure that all youth in the program have at least 3 or more Family Team Meetings with treatment team prior to youth’s exit from the program.

- Coordinate with youth therapist to ensure youth is attending weekly therapy sessions.
- Supervise Independent Living Coordinator to ensure daily duties are complete (i.e case plan goals, weekly schedules, budgeting, life skills, groups, and documentation of services).
- Ensure Independent Living Coordinator enrolls all youth in school within in 72 hours of entering the program.
- Monitor and confirm shift changes are complete at the beginning of each shift
- Monitor YES Team Staff to ensure food is ordered for the house weekly
- Maintain and/or ensure upkeep of the house in accordance with the Department of Health Services as well as agency policy.
- Perform other related duties as assign

Minimum Qualifications
- Master’s Degree in Social Work preferred; Bachelor’s Degree in Social Work required.
- Prior working experience and knowledge with homeless, runaway, foster and/or probation youth
- 1-2 years prior experience working with vulnerable youth in numerous settings preferred.
- Training in Trauma Informed Care, Crisis Intervention and Positive Youth Development preferred.
- Strong written and verbal communication skills. Outstanding organizational skills.
- Ability to manage multiple priorities and meet deadlines in a timely manner. Demonstrate problem solving ability.
- Experience working as part of a team, while also demonstrating initiative and ability to work independently.
- Satisfactorily fulfill the requirements for employment at the Youth Emerging Stronger i.e. references, physical examination, criminal records clearance, etc.
- Bi-lingual Spanish Preferred.
- Certificates/Licenses: Valid CA Driver’s License (must pass insurance underwrites)

Physical Demands:

- Sitting at desk with intermittent walking, standing, bending, and occasionally lifting light objects.

Pay: $57,000.00 - $63,000.00 per year
