This professional social services position is assigned to the Community Support Services Unit. This caseworker will provide a wide array of cross-agency services to students and their families in a variety of settings. Responsibilities include assessment of needs, short-term interventions, referral to appropriate community services and follow-up. This position will act as the mental health liaison to the Student Assistance Program Teams in assigned schools. The position will provide services intended to reduce the need for more formal agency intervention. This position is supervised by the Community Support Services Supervisor.

**Essential Functions**
- An individual must be able to successfully perform the essential functions of this position with or without a reasonable accommodation._
- Provide information, assessment, referral, prevention and short-term intervention services for all identified children and families in the Montoursville School Districts.
- Serve as the SAPliaison for mental health assessments and follow-up services in the middle and high schools of the Montoursville School Districts and provide liaison services to Lycoming Children and Youth Services and Lycoming-Clinton Mental Health.
- Participate in individual and treatment team discussions to prioritize referrals, develop strategies, provide updates on existing referrals, and recommend transition plans.
- Conduct individual and/or group sessions for children and/or families based on identified needs.
- Work with the school, child and family to address identified concerns by providing short term intervention and referral to appropriate community services.
- Develop positive working relationships with existing community resources, coordinating, negotiating the provision of needed services for identified youth, and provide education to faculty, students and families regarding services.
- Maintain a flexible work schedule that is responsive to the child, family, school and community’s needs providing outreach services in natural settings such as the family home, school or neighborhood.
- Follow up on requests from the Joinder Assessment Services Unit to gather relevant information on students in the Montoursville Area School District.
- Assess and provide short-term intervention services for truancy cases referred by the School District.
- Keep logistic and statistical information on truancy cases and other service interventions for quality assurance, outcome measures, and continuous service improvement.
- Attend regularly scheduled staff meetings and appropriate trainings to further develop skills.
- Maintain working knowledge of all policies, procedures and regulations pertaining to the provision of child welfare and mental health services.
- Maintain all relevant information on students using the current client information system. This will include referral information, reason for referral, secondary problem codes, school information, student information, parent information, case notes, recommendations, and truancy numbers.
- Maintain appropriate records of contact, nature of service provided, outcome and follow-up needed along with any State reporting requirements and data input into the applicable State and/or Joinder computer systems.
- Maintain mandated client confidentiality.
- Assure Agency clients coordinated access to services and provide transportation to Agency clients when appropriate.
- Perform other related duties as requested.

- Enter, exit and move about multiple residential and office settings
- Travel to client’s homes/residences in order to serve and meet their needs in their own environments as appropriate.
- Operate standard office machines

Pay: From $47,548.80 per year
