Del Rosa Hospice is a growing hospice company, locally owned and operated, serving the Greater Palm Springs Area (Coachella Valley) and the Inland Empire. Our management team has over 20 years of combined hospice experience, and our team is committed to making a difference in the lives of the patients and families we serve by providing quality, compassionate care when it matters most.
The Community Liaison, in conjunction with other members of the admissions team, is responsible for activities and actions related to growing our business by promoting the services and expertise of hospice to the medical community, patients and family members and the communities at large.
**Functions and Responsibilities**:
- Identifies, develops and maintains referral source relationships
- Responds to inquiries and requests for hospice information in a timely manner
- Follows up on referrals for evaluation and/or admission to the hospice program
- Explains the hospice philosophy to patients, family members and referral sources
- Verifies and completes applicable admission and financial information on the referral intake form and coordinates with other team members
- Organizes and provides education to the community, referral sources and facilities
- Works in conjunction with the admissions team, as well as assist the admissions team in obtaining any outstanding admission documents and patient financial information, as needed.
- Attends and participates in in-services, staff meetings and case conferences.
- All other tasks and duties deemed necessary and appropriate.
- Required : at least 1 year of sales or healthcare experience
- Bachelor’s degree in healthcare, marketing or public relations preferred
- Current driver’s license, auto liability insurance and reliable transportation
- Knowledge of hospice requirements including Medicare, Medicaid and private insurance guidelines
- Strong sales and critical thinking skills
- Ability to build and maintain relationships with referral sources
- Ability to communicate effectively and work well with others
- Bilingual, preferred
- Passion for serving others
**Salary and Benefits of Full-Time Position**:
- Pay and Benefits Wage: $60,000 to $110,000
