Job Summary

We are seeking a highly skilled RN Clinical Liaison to join our team. As a RN Clinical Liaison, you will play a critical role in educating healthcare professionals about our hospice services and providing end-of-life solutions that are committed to clinical excellence, compassion, and dignity.

Key Responsibilities
  • Develop and maintain relationships with healthcare professionals and referral sources
  • Provide education and training on hospice services and benefits
  • Collaborate with interdisciplinary teams to ensure seamless care transitions
  • Conduct patient assessments and develop individualized care plans
  • Monitor patient outcomes and make adjustments to care plans as needed
Requirements
  • RN license in good standing
  • Minimum of 2 years of experience in hospice or palliative care
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
