We are seeking a highly skilled RN Clinical Liaison to join our team at Gentiva Hospice. As a key member of our team, you will play a vital role in ensuring the delivery of exceptional patient care and services.
Key Responsibilities- Develop and maintain relationships with healthcare professionals, hospitals, and other referral sources to promote Gentiva's hospice services.
- Provide education and training on hospice care and services to healthcare professionals and patients.
- Collaborate with the interdisciplinary team to ensure seamless patient care and services.
- Conduct patient assessments and develop individualized care plans to meet the unique needs of each patient.
- Ensure compliance with regulatory requirements and Gentiva's policies and procedures.
- RN license in the state(s) of employment.
- Minimum of 2 years of healthcare experience, preferably in hospice, home health, or clinical liaison roles.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
