**Job Summary:** 

The Community Care Coordinator at Willow Spring Hospice Care plays a crucial role in connecting patients, families, and the community with hospice services. This position is responsible for outreach, education, and coordination of care, ensuring that patients receive the appropriate support and resources. The Community Care Coordinator serves as a liaison between the hospice care team and the broader community, promoting access to hospice services and facilitating seamless transitions of care. 

 

**Key Responsibilities:** 

- **Community Outreach:** 

Develop and implement outreach strategies to raise awareness about hospice services within the community. Build and maintain relationships with healthcare providers, community organizations, and other key stakeholders to promote hospice care. 

- **Patient and Family Support:** 

Assist patients and families in understanding hospice care options and services. Provide guidance and support throughout the referral and admission process, ensuring a smooth transition to hospice care. 

- **Care Coordination:** 

Work closely with the interdisciplinary hospice team to coordinate care for patients, ensuring that services are aligned with the patient’s needs and goals. Assist with scheduling, resource allocation, and communication between care providers. 

- **Education and Advocacy:** 

Conduct educational presentations and workshops for community groups, healthcare providers, and the general public to increase understanding of hospice care and its benefits. Advocate for patients and families, ensuring they receive the care and support they need. 

- **Resource Management:** 

Identify and connect patients and families with appropriate community resources, including social services, financial assistance, and other support systems. Maintain a network of community partners to facilitate access to these resources. 

- **Documentation and Reporting:** 

Maintain accurate records of outreach activities, patient interactions, and care coordination efforts. Prepare reports on community engagement and referral trends to inform program development and improvement. 

- **Compliance and Ethics:** Ensure all outreach and care coordination activities comply with hospice regulations, company policies, and ethical standards. 

 

**Qualifications:** 

- **Education:** 

Bachelor’s degree in social work, nursing, healthcare administration, or a related field. Relevant experience may be considered in lieu of a degree. 

- **Experience:** 

Minimum of 2 years of experience in healthcare, social services, or community outreach. Experience in hospice, palliative care, or home health is highly preferred. 

- **Skills:** 

Strong communication (English & Spanish), organizational, and interpersonal skills. Ability to work independently and as part of an interdisciplinary team. Knowledge of community resources and healthcare systems.

 - **Other Requirements:** 

Valid driver’s license and reliable transportation. Ability to travel within the service area for community outreach and patient support. 

 

**Working Conditions:**

 - **Environment:** 

The position involves a combination of office work, community outreach, and patient interaction. Travel within the service area is required. 

- **Physical Demands:** 

The role may require standing, walking, and driving for extended periods. Flexibility to attend events and meetings outside of regular business hours.
