Position: Harm Reduction Navigator (Social Worker) Pay beginning at $27/per hour
The Harm Reduction Navigator Responsibilities will include:
1) Identify and build relationships through phone calls and in person visits to the local social service, health, education, recreation and law enforcement agencies that may support persons who use drugs and/or their family members, friends and employers.
2) Develop research guides for each County (Lancaster, Dauphin and York) to facilitate navigation of those in need to appropriate resources. The guide will be routinely updated through contacts with agency personnel and expressed needs of community members.
3) The navigator will identify areas within each County for focused outreach efforts by attending Community meetings, reviewing reports from County Coroners, law enforcement, participant feedback and agency requests.
4) The Navigator will visit agencies in the focus areas to learn of new needs, deliver educational sessions, educational materials, contact cards designed for their region, provide linkage to other agencies, distribute overdose prevention supplies, and inform staff of mail-based distribution of overdose prevention supplies.
5) In locations of known drug use activity, the navigator will set up tables and provide educational materials and overdose prevention supplies, including test strips and naloxone, and navigate participants to needed or requested services. When necessary and possible, the navigator may facilitate transport to medical or social service agencies.
6) The Navigator will identify and participate in community-based meetings, health fairs and deliver harm reduction messages through local news and outlets and community social media sites.
7) The navigator will assist with assembly and mail delivery of overdose prevention kits.
8) Each month the Navigator will review information provided by the county coroners, law enforcement, participant feedback, and other sources to prioritize locations for future outreach efforts.
9) The Navigator will collect information about encounters, questions asked, materials distributed, linkages made, and when possible, the result of linkage efforts.
10) Compile data from activities and Report Monthly to Director, and as requested related to grants.
