About the Role:

Fairfield Medical Center is seeking a skilled Social Service Worker to join our multidisciplinary team. As a key member of our healthcare team, you will provide psychiatric social work services to inpatient hospital clients, working closely with physicians and other healthcare professionals to develop patient-specific treatment plans.

Key Responsibilities:

  • Collaborate with physicians and other healthcare professionals to develop patient-specific treatment plans.
  • Monitor patients' clinical progress throughout hospitalization and provide ongoing assessment and consultation to meet patient and family needs.
  • Assess and provide educational instruction and materials to meet learning needs of patients and family members.
  • Complete psychosocial assessments and multidisciplinary treatment plans.
  • Assist with multidisciplinary assessment summary pages for all discharges.
  • Provide groups and individualized therapy as directed.
  • Facilitate patient transfers to other psychiatric facilities.
  • Coordinate with psychiatric physicians for judicial hearing needs.
  • Coordinate post-discharge needs.

Requirements:

  • Bachelor's in Social Work with approval of Ohio Department of Mental Health permitted.
  • Masters degree in Social Work preferred.
  • Licensed Social Work (LSW) with practice directed and supervised by attending Psychiatrist(s) permitted.
  • Licensed Independent Social Worker (LISW) preferred.
  • A minimum of three years of experience or combination of experience and education.

Physical Demands:

Exerting up to 20-50 pounds of force occasionally, and/or up to 10-25 pounds of force frequently, and/or a negligible up to 10 pounds amount of force often.
