We are seeking OUTREACH & ENROLLMENT SPECIALIST to become a part of our team You will provide appropriate outreach and service linkage activities to clients who are not enrolled in primary care or lack access to healthcare and health insurance.

**Responsibilities**:

- Conduct outreach activities to identify and screen clients for services needs
- Providing information and referrals and assistance with linkage to medical, mental health substance abuse and psychosocial services as needed.
- Advocating on behalf of clients to decrease service gaps and remove barriers to services.
- Helping clients develop and utilize independent living skills and strategies.
- Ensures timely and coordinated access to medically appropriate levels of health and support services and continuity of care.
- Develop service plan to implement and monitor of efficacy over a period of time.
- Linking clients with high need levels to case management
- Networking with testing sites, medical facilities, and services providers to establish a referral network.

**Qualifications**:

- BA/BS from an accredited college or university with a major in social services or behavioral sciences.
- Minimum of (1) year paid experience in the social services.
- Experience working with persons with HIV/AIDS, preferred.
- Computer proficient, excellent written and verbal skills.
- Excellent written and verbal communication skills.
- Detailed oriented and organized.
- Self
- motivated, ability to work independently
- Ability to work with frequent interruptions and maintain emotional control under stress.
- Bilingual (Eng/Spn), a plus

**Working Conditions**
- The position occasionally requires sitting, standing and bending. Occasional very light lifting, up to 20 pounds or an equivalent weight may be required
