The **Care Coordination Manager** is a crucial leadership position within our community health center, tasked with guiding the strategic and operational direction of the Care Coordination department. This role is pivotal in overseeing the delivery of patient-centered, high-quality healthcare, enhancing service efficiency, and ensuring compliance with healthcare standards. The manager will lead a team of Care Coordinators to ensure a seamless interface between patients and healthcare services, working closely with the Director of Patient Support Services to promote excellence in healthcare delivery across various service lines including Medicare supportive services, Annual Wellness Visits, prenatal and postpartum care, and immunizations for children 0-5 years of age.

**Core Duties and responsibilities, include but are not limited to**:

- Lead, support and mentor Care Coordinators to ensure operational efficiency and effective management of patient care services.
- Develop and implement policies and procedures to enhance service delivery, focusing on Medicare supportive services, Annual Wellness Visits, prenatal and postpartum care, and immunizations for children 0-5 years of age.
- Manage the oversight of Diabetes Management programs from the perspective of Care Coordination.
- Focused target of FTOC patient populations including those not seen in 6 and 12 months, addressing any relevant care gaps to patient populations that require attention.
- Coordinate new patient initial health assessments and facilitate access to internal FTOC specialty services.
- Provide general and clinical administrative support, ensuring high-quality health services are delivered both onsite and in outreach settings.
- Initiate and manage programs to enhance patient engagement and care continuity.
- Collaborate with peers and the Director of Patient Support Services to develop and implement operational plans for effective healthcare delivery from the Care Coordination perspective.
- Ensure accuracy in patient appointment scheduling, confirmation, and follow-up for required paneled patients.
- Facilitate comprehensive access to transportation and social services to support patient care by initiating referrals to services within the Care Coordination Support Team.
- Actively participate and represent the department in various organizational meetings and events.
- Build and maintain relationships with external stakeholders to enhance patient care and service referral processes.
- Work closely with the marketing and business development teams to effectively promote healthcare programs and services.
- Keep abreast of all organizational services, actively seeking training and updates on new programs as necessary.
- Promote professional conduct and foster a collaborative work environment among team members.
- Compile and review monthly productivity and performance reports with the Director of Patient Support Services.
- Oversee the management and logistics of equipment, and supplies.
- Undertake additional duties and responsibilities as assigned by the Director of Patient Support Services.

**Education and Training**:

- Bachelor’s degree in social work, nursing, health, human services or closely related field; Master’s Degree (preferred.) Experience will be considered in lieu of degree.
- Minimum four years of experience in the human services field. Additional experience may be substituted for the academic requirements identified above.
- Knowledge and experience in working with diverse communities in the Orange County, CA area.
- Thorough understanding of Medicaid Waiver systems and community resources.
- Bilingual: Spanish (required)

**Qualifications, Skills, and Experience**:

- Minimum four years of experience in the human services field.
- Must excel in multitasking within a high-paced environment.
- Strong computer skills, acute attention to detail, confident and professional communication.
- Responsiveness to the needs of both internal/external stakeholders in a professional and personable manner are expected.
- Knowledgeable in general EMR/EHR systems is preferred.
