**JOB REQUIREMENTS**

You Must Have:

- **One year of experience within the past two years of direct client care experience at a facility**, hospital, or home health agency UNDER THE SUPERVISION OF A NURSE _or have an active CNA certificate_.
- Driver’s license
- Reliable transportation to and from work - your own car
- Proof of auto insurance
- Original Social Security card or INS authorization to work in the United States
- Ability to pass a background check, driver’s record check and drug test
- Current TB test: skin test taken less than 12 months prior, chest x-ray taken less than five years prior
- Ability to work either a Saturday or Sunday every week
