**Program Description**:
ISC211+ provides easy access to accurate and comprehensive resource information & referrals for the residents of San Bernardino County, provides public information support during disaster, and answers several specialized lines, and manages a countywide Coordinated Entry System (CES) known as Pathways Home Project. The project serves people who are experiencing homelessness throughout San Bernardino County and undertakes any project serving our target population. The Housing and Disability Advocacy Program (HDAP) focus for this role is a more in-depth service to those with chronic disabilities experiencing homelessness. The Emergency Solutions Grant (ESG) focuses on serving “literally homeless persons” and “at risk of homeless persons” in need of referral to shelters, rapid rehousing, and homeless prevention referrals.

**Essential Functions**:

- Provide excellent customer service to people experiencing homelessness or a housing crisis, over the phone or in person.
- Case Manage HDAP clients in emergency motel or shelter and work with partners to case manage those not accepting shelter through HDAP.
- Satisfy client’s immediate needs such as food, clothing, hygiene articles, and other basic items.
- Provide clients preliminary assistance such as becoming “document ready.”
- Provide in-person assistance, including transportation (bus passes/gas cards), by performing weekly outreach; facilitate a smooth transition ("warm handoffs") to other service and housing providers.
- Record client information in various systems including, but not limited to, inContact, iCarol, HMIS, and Excel spreadsheets.
- Work closely with engagement teams, case managers, and housing navigators.
- Advocate with other agencies for individuals to receive necessary services and housing.
- Conduct client follow-up and document outcomes.
- Perform HDAP outreach by visiting clients in motels on a weekly basis, or as needed. This includes but is not limited to, notifying clients they must exit emergency motel, clear out motel rooms of client’s belongings, and notifying motel management of any immediate room issues. Additional outreach activities may be required based on need.
- Coordinate housing with clients, housing locators, and agency program staff.
- Identify available community resources and provide in-depth referrals to housing and medical and mental health services.
- Provide Housing Navigation on a case-by-case basis.
- Develop relationships with external partners and coordinate outreach events and/or efforts in the community.
- Respond to crises and to calls from stakeholders involving concerns about homeless people in need of assistance.
- Successfully maintain performance parameters and learn through various means to maintain appropriate knowledge of housing programs and services.
- Document cases through all appropriate systems and create case files for the Coordinated Entry System.
- Abide by HDAP, ESG, and HUD Policies and Procedures for implementation of services and guidance for required submissions.
- Take live calls and assess eligible persons with prescreening and VI-SPDAT tools. Such assessments may be done in person from time to time.
- Satisfy client’s immediate needs with resources for food, clothing, hygiene articles, medical and mental health services, and other basic needs.
- Attend team meetings, case conferences, training workshops, and community meetings, as assigned by supervisor or as needed.
- Other duties as assigned.

**Qualifications**:

- Experience working with the homeless community, performing outreach and/or providing direct services to individuals experiencing homelessness, preferred.
- Two years of prior comparable work experience.
- Ability to listen with sensitivity to other people’s feelings, needs, and point of view; demonstrate respect, tact, and courtesy in expressing options or ideas.
- Recognize opportunities to enhance community relations.
- Passion and desire to work towards ending homelessness.
- Patience to manage critical cases with chronic disabilities.
- Knowledge of local resources and HUD regulations preferred but not required.
- Knowledge of mental health and substance abuse clinical skills preferred but not required.
- Excellent writing skills required, including correct grammar and ability to summarize.
- Proficient computer skills
