Health Home Case Manager

The Case Manager will provide case management support for patients, including conducting assessments, developing care plans, and coordinating with healthcare and social services providers to meet patient needs.
1. Promote a safe, cooperative and professional health care environment to ensure optimum patient care (as per HR Policy 113c).
2. Adheres to all hospital policies, procedures, rules and regulations, including but not limited to, absenteeism, ETIME, cellular telephone usage, dress code and rules of conduct.
3. Makes Customer Service a priority, treating customers (patients, visitors, and co-workers) in a professional manner exercising courtesy and tact. Effectively communicates with patients and others about SBH Health System services in their area(s) of expertise or responsibility and ensures that concerns, questions or issues reach the right person(s) in SBH in a timely fashion.
4. Demonstrates the standards of performance and behaviors consistent with the DRIVE to Patient-Centered Excellence.
5. Gather information for and or conduct outreach, intake, assessment, and reassessments. Register all patients for potential admission to SBH programs/services when necessary.
6. Provide care management to a caseload through the coordination of medical, mental health and substance abuse services.
7. Implement activities to facilitate participant engagement.
8. Participate in case conferences with health home team including, Case Managers, treatment provider, and other members of a multidisciplinary team.
9. Ensure entitlements, insurance, and benefits are in place
10. Develop service plans and resolve barriers to effective service utilization.
11. Actively participate in training opportunities to stay current or develop skills with the changing systems including Health Home Plus trainings.
12. Monitor client's progress in utilizing services (appointments, treatment, medication, etc.) through telephonic and direct contact
13. Document service information in databases and use electronic systems to track participant activities.
14. Conduct home visits and community work in the field. Timely follow up with hospital/ER upon notification of a client’s admission and/or discharge to/from ER/hospital/residential/rehabilitative setting required. Visiting hospitals and emergency rooms when determined necessary. Escort clients to/from medical appointments when needed.
15. Ensure that clients follow-up with aftercare discharge (i.e. fill prescriptions, make appointments). Check that clients receive test results and track that clients follow up with medical directions.
16. Case managers will be responsible for completing all aforementioned duties including others as assigned to patients who have been identified as Health Home Plus.
17. Coordinate and sustain connection of patients with appropriate SBH resources including Health & Wellness, Population Health, Health Home, care management, other special programs, and community resources.
18. Translate or facilitate translation as necessary
19. Other duties as assigned
Pay: $50,000.00 - $55,000.00 per year
