About the Opportunity

We are seeking a skilled Case Manager to join our team at Tandym Group in the Bronx, New York City. As a Case Manager, you will be responsible for providing direct service provision to residents, including individual counseling, group counseling, advocacy services, referrals, and follow-up, home management, and financial counseling.

Responsibilities
  • Maintain a caseload of 25-30 families and conduct intake interviews to assess and evaluate residents for admission using the DHS CARES system.
  • Engage residents in initial and ongoing treatment processes through individual and group counseling, under the clinical supervision of the Social Work Supervisor.
  • Conduct independent living plans on a biweekly basis in the DHS CARES system and monitor case plans for effectiveness and individual client needs.
  • Develop and maintain housing referral resources and assist residents in locating and securing permanent housing, in conjunction with the Housing Specialist.
  • Assist residents in the process of referral and placement in community-based resources, including aftercare services.
  • Assess residents' vocational and educational needs, aptitudes, and interests through interviews and testing, and develop and maintain placement and follow-up as appropriate.
  • Develop and maintain liaison activities with DHS, HRA, Public Assistance, and Medicaid Administrators and Workers to assist eligible residents in securing Public Assistance and Medicaid.
Qualifications
  • 2+ years of experience working with the Homeless population.
  • Associate's and/or Bachelor's Degree.
  • Familiarity with DHS regulations and DHS Cares Reporting.
  • Experience in assessing vocational and educational needs and aptitudes of clients, thorough familiarity and success with engaging clients, and assisting eligible clients through the process of obtaining Public Assistance, Medicaid, and other benefits.
  • Experience with supportive group facilitation.
  • Demonstrated sensitivity and experience with and/or knowledge of homelessness within disenfranchised communities, including individuals who identify as LGBTQ+ and/or HIV-affected, the elderly, and individuals with substance abuse and/or mental health issues.
  • Demonstrated skills in group counseling.
  • Microsoft Office proficient.
  • Great interpersonal skills.
  • Excellent communication skills (written and verbal).
  • Strong attention to detail.
  • Highly organized.
