Job Summary

We are seeking a highly skilled and experienced Health Home Supervisor to join our team at St. Barnabas. As a key member of our management team, you will be responsible for overseeing the Health Home Census, managing Case Managers, and promoting effective education, self-management support, and timely healthcare delivery.

Key Responsibilities
  • Manage the Health Home Census and individual Case Managers to ensure timely and effective care coordination.
  • Promote effective education, self-management support, and timely healthcare delivery to achieve optimal quality and financial outcomes.
  • Behave in a professional manner and consistently demonstrate and promote the values of respect, honesty, care, and dignity for patients and healthcare team members.
  • Collaborate with SBH Behavioral Health and Medical providers to improve coordination of services and communications.
  • Supervise the referral process to ensure timely processing and enrollment.
  • Provide direction, support, and supervision to Case Managers.
  • Hold periodic staff meetings to keep members current with procedural and policy issues.
  • Monitor worklists for each Health Home staff member to ensure timely documentation and procedural compliance.
  • Review and approve documentation as required.
  • Provide oversight and instruction to ensure coordination and linkage to behavioral health, medical, and support services.
  • Act as a conduit between Health Home, Behavioral, and Medical providers to ensure client needs are met.
  • Provide crisis management coordination with providers and staff as needed.
  • Ensure timely completion and submission of all required incident documentation.
  • Responsible for training of new employees.
  • Meet with patients, primary care, and behavioral health providers to develop and coordinate treatment plans.
Requirements
  • Masters Degree in Social Work or related field.
  • 2 years supervisory experience in a behavioral health setting.
  • Three years' experience in care management with progressive responsibility.
  • Ability to interact with all levels of staff in a professional manner.
  • Proficient in Microsoft Office, Excel, EMR.
  • Strong written and verbal communication skills.
  • Bilingual - Spanish/English (preferred).
