Job Summary:

The Home Health Nurse will assess, plan, implement, and evaluate patient care plans in collaboration with the patient, family, and healthcare team. The ideal candidate will have a strong background in nursing, excellent communication skills, and the ability to work independently in a fast-paced environment.

Responsibilities:
  • Provide individualized skilled nursing care to patients in their homes
  • Assess patient needs and develop a plan of care in collaboration with the patient and healthcare team
  • Implement and evaluate patient care plans, making adjustments as needed
  • Coordinate services with other healthcare professionals and agencies
  • Document patient care and progress in a timely and accurate manner
  • Participate in interdisciplinary team meetings and case conferences
  • Collaborate with case managers and other disciplines to ensure seamless care transitions
  • Provide education and support to patients and families on disease management and self-care
Requirements:
  • Licensed and registered to practice as a professional nurse in New York State
  • Current certification in Basic Life Support procedures, including CPR
  • Baccalaureate degree in Nursing from an approved program preferred
  • Minimum two years of full-time nursing experience, with at least two years in Homecare, Long-term Care, or Hospital setting
  • Excellent verbal and written communication skills
  • Ability to work independently and as part of a team
