Intake Coordinator, Home Health

**About Acacia**:
**Responsibilities**:

- Processing home health patient referrals
- Identifying insurance coverage
- Assisting case managers in managing patient care services
- Coordinating admission of home health services with patients and referral sources
- Providing resource information options, and collaborating with the team

**Qualifications**:

- Minimum of one year of experience in intake coordinating required
- Home health experience preferred
- HomeCare HomeBase experience preferred
- Strong knowledge of medical terminology
- Must have excellent interpersonal & customer service skills
- Ability to work well a fast-paced environment and multi-task
- Proficient with using a PC, Microsoft Word, Excel, and EHR

Pay: $23.00 - $25.00 per hour
