Job Summary:
We are seeking a highly motivated and results-driven individual for the role of Regional Director of Care Transitions at Paradigm Healthcare, LLC in Houston, Texas. This is a full-time, individual contributor role in our marketing department. As the Regional Director of Care Transitions, you will be responsible for developing and implementing strategies to increase the number of patient admissions and transitions from acute care facilities to our skilled nursing facilities.

Compensation & Benefits:
This position offers a competitive salary range of $75,000 to $85,000 per year, paid biweekly. In addition, we offer a comprehensive benefits package that includes health, dental, and vision insurance, 401(k) retirement plan with company match, paid time off, and opportunities for career advancement.

**Responsibilities**:

- Develop and implement effective strategies to increase patient admissions and transitions from acute care facilities to our skilled nursing facilities in the Houston region
- Build and maintain relationships with key stakeholders in hospitals, rehabilitation centers, and other healthcare facilities
- Collaborate with facility managers and clinical teams to ensure smooth and timely transitions for patients
- Conduct market research and analysis to identify potential growth opportunities and competitive threats
- Develop and deliver presentations to healthcare providers and community organizations to promote our services and build brand awareness
- Monitor and report on key metrics and outcomes related to patient transitions and admissions
- Participate in industry conferences and events to stay updated on industry trends and best practices
- Collaborate with the marketing team to develop marketing materials and campaigns that support care transition efforts

**Requirements**:

- Bachelor's degree in Healthcare Administration, Marketing, Business, or a related field
- 3-5 years of experience in healthcare sales, business development, or marketing, preferably in the post-acute care space
- Strong understanding of the healthcare landscape, particularly in the Houston region
- Proven track record of developing and maintaining relationships with key stakeholders in healthcare organizations
- Excellent communication, presentation, and negotiation skills
- Ability to analyze market trends and use data to inform decision-making
- Demonstrated ability to work independently and achieve results in a fast-paced and dynamic environment
- Willingness to travel within the Houston region as needed
