This role requires a compassionate professional dedicated to helping individuals navigate hospice care's emotional, psychological, and practical challenges. You will work closely with the hospice care team to ensure comprehensive and compassionate care for all patients.

**Key Responsibilities**:
**Counseling**:

- Provide counseling services to clients and their families, addressing perceptions of the dying process and offering grief counseling.
- Mediate conflicts within families and help them define personal goals for hospice care to facilitate decision-making during difficult times.

**Education**:

- Educate clients and their families on coping strategies and techniques to manage pain and other symptoms of illness.

**Assistance with Paperwork**:

- Assist in reviewing and documenting clients' end-of-life wishes, including Do Not Resuscitate (DNR) orders and advanced directives.
- Help clients navigate medical or veterans benefits and advise on financial or legal matters, including Medicaid/Medicare paperwork.

**Assessing Challenges and Risks**:

- Observe and assess clients' environments to identify potential challenges and risks such as abusive or neglectful situations, emotional needs, and safety risks.
- Collaborate with the hospice care team to ensure all patients receive comprehensive care.

**Qualifications**:

- Bachelor’s degree in social work, psychology, or sociology (master’s degree preferred).
- State licensing in social work is required.
- Certification in social work or hospice care (e.g., Certified Hospice and Palliative Social Worker - CHP-SW) is highly desirable.
- Minimum of three years of documented supervised social work experience in hospice and palliative care.
- Strong counseling and mediation skills.
- Excellent communication and interpersonal skills.
- Ability to handle sensitive and emotional situations with compassion and professionalism.

**Work Environment**:

- This position involves working closely with clients and their families in a variety of settings, including homes and hospice facilities.
- The role requires travel within the service area to visit clients and collaborate with healthcare teams.

**Job Types**: Part-time, Contract

Pay: $55.00 - $85.00 per hour
