We are seeking a compassionate and detail-oriented Registered Nurse (RN) to join our Hospice of Chattanooga team as a Case Manager. As a key member of our interdisciplinary team, you will play a vital role in providing exceptional end-of-life care to our patients and their families.
Responsibilities- Assess patient needs and develop personalized care plans in collaboration with our Clinical Director.
- Coordinate and deliver high-quality care, ensuring the comfort and quality of life for our patients.
- Provide education and support to patients and families, empowering them to take an active role in their care.
- Participate in call duties, demonstrating our commitment to being there when it matters most.
- Current unencumbered license as a Registered Nurse.
- Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice experience a plus.
- Computer proficiency in typing and various programs, including background in EMR.
- CPR certification if required by state.
