SERVICE AREA - Essex and Union Counties
The NJTCO Support Coordinator is an organized, motivated person who works well in both independent and team settings. The Support Coordinator will manage services for individuals with intellectual/developmental disabilities.
**Support Coordinator Responsibilities**:
- Develop Individualized Service Plan (ISP) and Person-Centered Planning Tool (PCPT) for individuals enrolled in DDD/Medicaid services.
- Assure the information in ISP and PCPT is understandable and written in a person-centered manner.
- Connect the individual with services and supports; reach out to providers to confirm service capacity, intake/eligibility requirements; gather and submit referral information as needed.
- Conduct monthly monitoring at specified intervals and revise the ISP as needed.
- Develop a thorough understanding of the services funded by DDD and other state and community resources, to ensure that individuals/families are offered informed choices of services and providers.
- Ensure delivery of quality services and ensure that services are provided in a safe manner, in full consideration of the individual’s rights.
- Adhere to deadlines - submit new plans within 30 days of enrollment; conduct plan renewal meetings at least 60 days prior to renewal. Submit plan renewal to Supervisor at least 30 days prior to plan renewal date.
- Maintain monthly contact, or more frequently as needed. Complete required documentation via the Monthly Monitoring Tool. _Monthly contact via telephone, face-to-face or home visit that includes review of the ISP and documentation on the Monthly Monitoring Tool._
- Maintain accurate case notes in the iRecord in a timely manner.
- Maintain a confidential case record that includes, but is not limited to, the NJ Comprehensive Assessment Tool (NJ CAT), Monthly Monitoring Tools, PCPT, ISP, notes/reports, annual satisfaction surveys, and other supporting documents uploaded to the iRecord.
- Complete at least 12 hours per year of continuing education training.
**Experienced Support Coordination Responsibilities**
- Attend Monthly Staff Meetings.
- Participate in On-Call Rotation
**Required Knowledge/Skills/Attributes**:
- Experience and proficiency in using Office 365 products - Word, Outlook, Microsoft Teams, Excel.
- Strong and effective spoken and written communication skills.
- Respect and appreciation for people with disabilities, their families, other organizations, agencies, volunteers, and consumers.
- Able to work independently with mínimal direction and reach out to the NJCTO team when guidance is needed.
- Represent NJTCO in a professional demeanor and presentation.
**Must have own transportation, a valid driver’s license, and insurance, or access to reliable transportation.**
**Education and Experience**:
- Bachelor’s Degree or higher in any field - and
- 1 year of experience working with adult (18 or older) individuals with developmental disabilities. _The experience must be the equivalent of a year of full-time documented experience working with adults (18 or older) with intellectual/developmental disabilities. This experience can include paid employment, volunteer experience, and/or being a family caregiver of an adult with a developmental disability._
- State, Federal Criminal Background checks and Central Registry Check at the time of hire;
- and
- Successfully complete trainings required by the Division before rendering services
- College of Direct Support Trainings
**Working Conditions**:
- Standard Work Week is 9:00 AM - 5:00 PM - yet requires flexibility to meet the requests for consumer/families meeting after standard working hours.
- Requires in-home visits and attendance at community-based meetings, therefore Support Coordinator must have reliable transportation.
