Community Health Prep Coordinator

BASIC FUNCTION:
The PrEP Coordinator will facilitate center activities, resources, events and services regarding PreExposure Prophylaxis as an effective HIV prevention method. The Coordinator will increase the awareness of PrEP to high-risk individuals; expand the directory of culturally-competent medical providers prescribing PrEP; and provide direct linkage to care for people seeking access to PrEP as well as link newly diagnosed clients to the ID clinic.
This position is an integral part of our HIV/AIDS Program. This position is responsible for completing all elements as defined below. The position will be based out of a designated health clinic.
SUPERVISORY ACCOUNTABILITY:
Responsible for providing limited supervision for one or more functions within a department. Formally plans, assigns, directs, and coordinates the work of these functions. Typically responsible for performing some non-supervisory duties in addition to supervisory responsibilities. May perform staff evaluations and make recommendations regarding pay and/or performance
NATURE AND SCOPE:
Internal contacts include administrators, faculty, and staff both within the department and University-wide. External contacts include government agencies, sponsor representatives, vendors, visitors, officials and the general public.
PRINCIPAL ACCOUNTABILITIES:
- Increase the awareness of PrEP and nPEP among high-risk individuals
- Develop and expand online directory of educated, culturally-competent medical providers prescribing PrEP
- Provide basic non-clinical linkage-to-care and patient navigation for individuals seeking PrEP as a practical prevention tool
- Organize and facilitate new client orientations for PrEP
- Work with EIS and Outreach Coordinator to collect data on opt-out testing on a monthly basis
- Link newly diagnosed clients from opt-out testing to ID clinic
- Maintain opt-out testing program to test at a minimum 5,000 HIV tests by March 31, 2025
- Collect and evaluate reports from clients to ensure access to PrEP and follow-through with service providers
- Organize the planning and deployment of a creative media plan to educate the public about HIV risk and PrEP/nPEP options through our Director of Marketing
- Utilize editorial content, paid advertising, PSAs and social media platforms to raise awareness of PrEP
- Expand The Center’s printed and online educational materials on PrEP.
- Ensure the collection, collation and organization of resource, pamphlet, electronic article, and reference information and materials regarding PrEP
- Monitor the Confidential Histories section in the Electronic Health Record to identify those at high risk for HIV and more likely to benefit from a PrEP Coordinator consultation
- Promote and participate in related community and health events as needed
- Develop close working relationships with leadership and direct staff at other prevention clinics to ensure coordination of services
- Adhere to strict procedures of confidentiality, release of information and data security
- Provide HIV Counseling, Testing and Referrals as needed to at-risk populations
