Program (CBAP).

**Hours**:35 Hours/Week; Non-Exempt (Mon - Fri, 10AM - 6PM)
Occasional evenings for outreach events.
**Salary**:$45,000-$53,000/Year

**Major Duties**:
Direct Services:
- Conduct respectful, accurate, and timely intake, assessment, and service planning with all assigned program participants, entering information into required databases as indicated.
- Work in an ongoing manner with participants to provide housing benefits screenings, direct services, and referrals to address their immediate needs as well as longer-term concerns in order to help them achieve their goals and enhance their self-sufficiency.
- Provide low-income individuals and families in Crown Heights and the surrounding Brooklyn communities that are struggling with housing insecurity with a myriad of targeted housing support services so that they can achieve housing permanence and avoid placement in the shelter system.
- Enhance housing supports through helping individuals and families access wraparound services like job placement assistance, Medicaid, Medicare, health insurance subsidies, SNAP benefits, and more to remove barriers that often lead to financial, life, and housing instability.
- Provide advocacy, outreach, and referrals to entitlement agencies, social service providers, and other programs on behalf of program participants.
- Refer participants to in-house legal and financial consultants as needed, scheduling them in shared calendars.
- Develop ISP plan with short
- and long-term goals for participants enrolled in the program.
- Follow-ups: bi-weekly follow-up required in the program
Outreach:
- Conduct outreach in Crown Heights to promote the program and recruit new participants.
- Distribute promotional materials; attend community board, police precinct, and other neighborhood meetings; and use participant and staff word-of-mouth to keep their networks informed of services offered.
- Develop partnership with local community-based and city agencies to connect participants to needed housing services.
Pop-Up Events:
- Host at least 12 popup events (including at least six in Crown Heights) to assist attendees with housing supports and other resources and services.
Workshops:
- Schedule and facilitate at least 12 workshops annually (at least one per month) that focus on different housing related and other government benefit topics.
- Collaborate with expert workshop presenters as well as interested workshop participants to achieve maximum utilization.
Database-data entry:
- Use required tracking and reporting databases in order to document services provided, outcomes, and submit proof of goal achievement.
- Provide program updates at internal meetings.
- Maintain attendance and participation records and upload these into required databases in order to satisfy funder mandates.
- Compile data and complete statistical and written reports as needed.
- Perform other duties as assigned.
**Qualifications**:
- Bachelor’s degree required in relevant field; counseling credential preferred.
- At least three years of case management experience in a community-based setting preferred.
- Bilingual English/Spanish OR English/Creole required.
- Must have good written and verbal communication skills.
- **_ Must be able to work evenings and occasional Saturdays_**.
- **_ Must be open to work in Crown Heights and being in the field at least two times a week or more_**.
- Must have a commitment to work from a strength-based and youth and family development perspective.
