**JOB SUMMARY**:Under the direct supervision of the Program Manager, the Case Manager/Housing specialist for our DOHMH NYC 15/15 Adults and families is responsible for intake, monitoring, and maintaining single adults and/or couples with clinical conditions in our scatter-site supportive housing.

**DUTIES AND RESPONSIBILITIES**:

- Conducts intake assessments on new referrals for potential program admission and determines which services and assistance are necessary
- Coordinates with Senior Property Manager move-in for new clients and relocation for existing clients when necessary.
- Assists clients with establishing utility services and monitors monthly payments to ensure utilities are not disconnected for non-payment.
- Provides supportive counseling utilizing harm reduction and motivational interviewing approaches.
- Provides crisis intervention, de-escalation, and conflict resolution services.
- Conducts quarterly re-assessments, service plan development, and bi-annual mandatory agency forms with each client on a caseload.
- Monitors clients' progress toward their goals via regularly scheduled telephone contact and/or face-to-face visits, and document via progress notes.
- Assists with Wellness Self-Management activities.
- Provides individual tobacco smoking cessation sessions with clients.
- Calculates, collects, and tracks monthly rent payments from each client to ensure each client makes appropriate monthly rent payments and manages delinquencies as appropriate.
- Facilitate apartment repair/maintenance needs via BAC maintenance staff and advocacy with clients’ superintendents, building managers, and landlords.
- Refers clients to appropriate services such as medical, dental, substance use treatment, mental health treatment; benefits programs such as SSI, SNAP, legal, education, training, employment, and other services, and follow-up with the outcome of referrals.
- Other related duties as required or assigned based on program needs.

**KNOWLEDGE, SKILLS, AND ABILITIES**:

- Knowledge of issues related to substance use, recovery, and mental health.
- Excellent verbal, written, and interpersonal skills.
- Excellent organizational and record-keeping skills, as well as the ability to manage multiple cases.
- Computer literacy to maintain and manage case records.
- Sound judgment, critical thinking, problem-solving skills, and ability to multitask.
- Knowledge of fair housing regulations and laws pertaining to housing assistance programs
- Strong communication and interpersonal skills to effectively work with diverse populations
- Ability to assess client needs and provide appropriate support and resources
- Experience in crisis management or addiction counseling is beneficial

**EDUCATION AND WORK EXPERIENCES**:

- **MINIMUM**:Bachelor’s Degree in Social Work, Human Services, or related field OR 4+ years experience providing housing and/or case management services to the homeless population.

**Job Types**: Full-time, Temporary

Pay: $24.73 per hour

Expected hours: 35 per week
