**Standard work schedule**: Monday-Friday 8am-4pm

**Who We Need**

**MINIMUM QUALIFICATIONS**:

- Requires a high school diploma or GED.
- Must have one-year experience in behavioral health, rehabilitation, or employment services.
- Satisfactory work record as determined by the People First/Human Resources Department.
- Successful completion of Copa’s training program.
- Must have a fingerprint clearance card issued by the AZ Department of Public Safety or be eligible to receive one within eight (8) weeks of hire (If a Good Cause Exception is required, this must be taken care of BEFORE hire date.)
- Successfully complete a pre-employment drug screening.
- TB Test clearance required at the time of hire and annually.
- Must be an authorized driver, which includes a valid AZ Driver’s License, state minimum insurance coverage and the ability to meet and maintain Copa Health’s acceptable driving requirements.

**JOB SUMMARY (INCLUDING, BUT NOT LIMITED TO)**
- Ensure the maintenance of departmental operations to achieve compliance with contract requirements, accreditation, licensing and monitoring standards, agency procedures, Federal/State/Local regulations and all other licensing/regulatory agencies as directed. Adhere to current training requirements and applicable accreditation and licensing/regulatory standards and compliance with the Health Insurance Portability and Accountability Act (HIPAA) and Health Information Technology for Economic Clinical Health (HITECH ) requirements.
- Coordinate information and contribute to the successful resolution of challenges. Communicate with integrity and honesty, clearly and concisely, and in a respectful manner to all team members. Ensure the confidentiality of written, verbal and electronic communications pertaining to members, staff, families of consumers and agency operations.
- Demonstrate and maintain professional, respectful, productive working relationships with all agency departments, staff, board members, funding sources, individuals receiving services, families utilizing or enquiring about services, funding sources and the community.
- Engage members and establish trusting, collaborative relationships directed toward the goal of competitive employment in community job settings. Jobs that are “not” designated for people with disabilities.
- Assist members in obtaining individualized information about how entitlements (e.g., SSI, SSDI, Medicaid, EBT, etc.) will be affected by employment so members can make informed decisions about employment opportunities. This includes utilizing the DB101 website and interpreting session results accurately. Refer members to Ticket to Work if they choose.
- Encourage and assist members with completing a Career Interest Profile via O*Net to evaluate areas of interest.
- Complete a Vocational Profile to obtain member’s past work history and current employment goals.
- Complete a First Employer Contact with member within 30 days of Vocational Profile.
- Discuss member’s preference for disclosure of mental health status to employers.
- Conduct job development and job search activities directed toward positions that are individualized to the interests and uniqueness of the members on his/her caseload.
- Provide individualized follow-along supports to assist members in maintaining employment.
- May negotiate job accommodations and follow-along contact with the employer as agreed upon by members.
- Provide timely interventions. Returns phone calls and react to situations in a timely manner. For example, returns member phone calls within 24 hours.
- Meet with job searching members 1-4 times per month, depending on member’s abilities, skills and desires.
- Meet weekly with members who have obtained employment for the first 30 days, then taper down but to no less than monthly meetings until member is able to build up natural supports, maintain employment on their own and has requested closure.
