The Housing Coordinator will coordinate housing efforts for the Housing Program by developing and maintaining relationships with landlords and private management companies who will work with our clients. They will collaborate with different teams/programs, and with representatives from the city, state, and county to secure safe and affordable housing for clients. The Housing Coordinator will also support with all aspects of the United States Department of Housing and Urban Development (HUD)/Housing First grant, Projects for Assistance in Transition from Homelessness (PATH) Permanent Supportive Housing (PSH) grant, Ogden Apartments, and assigned as the Agency Technical Administrator. The Housing Coordinator will provide on-site case management services at Supported Residential Sites and in clients homes. The Housing Coordinator will attend Continuum of Care meetings and other assigned state and local events and committees as needed.

**Responsibilities**:

- Develop and maintain relationships with landlords and private management companies throughout the city to secure safe, affordable, and fair housing for clients.
- Coordinate with key stakeholders to Identify participants, locate housing units, manage participants to successfully maintain housing in accordance with the housing first model of care, database HUD/Housing First grant, PATH PSH grant, Supportive Grant, and SRN.
- Member of the Continuum of Care (CoC) committee to help meet requirements of the HUD grant. Actively participates in CoC meetings as the voting representative for the organization.
- Meet with clients to provide direct and supportive services related to housing, i.e., lease negotiation process, move-ins, and ensure clients are added to city and state referral lists for subsidized housing.
- Prepare and report information to state and local authorities as it relates to program benchmarks and standards.
- Trained Skilled Assessor with the ability to sign up homeless clients for the Coordinated Entry System (CES), Statewide Referral Network (SRN) for subsidized emergency housing, and the Homeless Management Information System (HMIS).
- Complete data entry in the HMIS and SRN systems for referrals to Ogden Apartments, Clark/Estes, and other residential sites.
- Provide support and education to teams regarding housing issues and help secure housing for their clients by developing and conducting teams-based training and information sharing sessions around housing searches and maintaining the SRN.
- Communicate and collaborate with class member programs and outreach teams to enhance client care, service delivery, and facilitate program operations and objectives. Provide new housing opportunities to teams seeking new leads.
- Perform other related duties and/or projects as assigned.

Qualifications:

- Must have one of the following requirements:

- Bachelor's degree in counseling and guidance, rehabilitation counseling, social work, education, vocational counseling, psychology, pastoral counseling, family therapy, or related human service field
- Bachelor's degree in any other field with two years of supervised clinical experience in a mental health setting
- Licensed Practical Nurse under the Nurse Practice Act [225 ILCS 65]
- Certificate of Psychiatric Rehabilitation from a DHS-approved program, a high school diploma/GED, and two years’ experience of experience in a mental health setting
- CRSS certification through IAODAPCA
- Family Partnership Professional certificate from and in good standing with IAODAPCA
- Occupational Therapy Assistant licensed under the Illinois Occupational Therapy Practice Act [225 ILCS 75] with one-year experience in a mental health setting;
- High school diploma or GED and five years of supervised clinical experience in a mental health setting
- Valid Illinois driver’s license with daily access to a well-maintained vehicle with $1,000,000/$3,000,000 liability insurance
- Experience working with people with mental health and/or substance abuse diagnoses preferred
- Experience working in the community, advocacy efforts, and marketing skills preferred
