Coordinator Discharge

**Job Overview**:
The Discharge Planner will provide discharge planning tasks and services to patients and their families, and support case managers and social workers. Responsibilities include responding to directives to complete basic patient assessment, complete referral processes to community agencies such a Board of Social Services, Meals on Wheels, and Adult Area on Aging, confirming a bed at a receiving post-acute facility, conveying insurance authorizations when needed, and arranging transportation to facilities. The discharge planner will facilitate the process of delivery of IM (Important message from Medicare), Observation Letters, and provide Advance Directive forms upon request.
**Qualifications**:
- Required:_
- Master's Degree
- Successful completion of all Orientation Programs
- Preferred:_
- One year experience in related field in a hospital setting
**Scheduling Requirements**:
- Day Shift
- Full-Time
- 40 hours per week
**Essential Functions**:
- Provides basic intervention to individuals and families, to enhance coping skills with the patient s/family s decision-making, reaction to the discharge and post hospital care needs. Will communicate with Social Worker to further assist patient/caregiver
- Interviews patients, family members and significant others to obtain relevant information
- Provides information to patients, families and interdisciplinary team members regarding available community resources and other post hospitalization options and make referrals in accordance with patient choice
- Performs other duties as assigned
