Job Title: RN-LPN-SNF Coordinator

Southcoast Health System is seeking a skilled RN-LPN-SNF Coordinator to join our team. As a key member of our post-acute care team, you will be responsible for coordinating bed offers with skilled nursing facilities (SNFs) to ensure seamless transitions for our patients.

Key Responsibilities:

  • Communicate with SNFs to secure bed offers for Southcoast Hospitals
  • Collaborate with case managers to facilitate patient transfers from acute care to SNFs
  • Manage the referral-to-bed-offer process, ensuring timely and efficient patient discharges
  • Develop and maintain strong relationships with SNF accounts to optimize bed availability
  • Document bed offers, bed flows, and discharge calendars to inform access to care

Requirements:

  • Current licensure by the Board of Registration in Nursing (LPN or RN)
  • 1-3 years of nursing experience, with at least 2 years in an SNF or sub-acute care facility
  • Administrative/admissions experience preferred
  • Excellent interpersonal, customer service, and communication skills
  • Proficient computer skills, including experience with various software and electronic records programs
