*SNF - LPN Coordinator Job Responsibilities:*

_*Monday-Friday 7:00-3:00 with 1 weekend ON CALL a month*__- _*Up to a $10,000.00 Recruitment and Retention Bonus*

Day Unit Supervisor – round on the floor –several times throughout the shift, monitor call bells, complete A&I(s), holding keys, monitor documentation of CNAs. Address all concerns on the floor and include RN manager and DON as needed
* Address resident needs and refer to RN manager if further orders or assessment is needed
* Completion of audits as needed
* Assist RN manager with admissions/discharges as needed
* Assume charge nurse duties at absence of charge nurse
* Assist nurses and CNAs daily
* Address family/resident concerns daily and include RN manager and DON as needed
* Schedule appointments if needed
* Provide coverage for nurse call offs as needed (RN manager/DON/LPN coordinator to rotate. Holiday coverage is required).
* Staffing huddles with RN manager and DON
* Manage showers/ weights and assure completion daily
* Assist with meals/feeds at every meal and assist with hall trays/answering call lights
* Training and orientation of new staff
* Monitor completion of health care academy for all staff
* Greet and monitor 2pm shift arrival –give report at beginning of shift.
* Monitor daily breaks of all staff for signing out correctly
* Run reports and monitor completion of health care academy/ input new employees into health care academy if needed

*EXPERIENCE:*

Must possess, as a minimum, a Nursing Degree from an accredited college or university, or is a graduate of an approved LPN/LVN program. Must possess a current, unencumbered, active license to practice as an LPN in New York State.

*EDUCATION REQUIREMENTS:*

Must have, as a minimum, 3-5 year(s) of experience as a Licensed Practical Nurse in a long-term care facility or other related health care facility. Must have training in rehabilitative and restorative nursing practices.
