Job Description
Job Description:As a Quality Care Coordinator, you will conduct patient outreach for management and coordination of care for chronic medical conditions and preventative health screenings utilizing patient registries. You will review medical records and CareEverywhere for documentation to provide complete medical records for patients. You will also assist with quality campaigns related to cancer screenings, immunizations, and other care gaps as needed. This position is fully remote, and will work 40 hours/week with a typical schedule of 7:00AM - 3:30PM. Though this position is remote, preference may be given to candidates who live within our service areas.
Education Qualifications:

Key Responsibilities:

  • Conduct patient outreach via letter, MyChart or Telephone to patients with open care gaps or other care needs

  • Review medical records and other healthcare data (ie. CareEverywhere), results and or reports for quality care gap closure

  • Work patient registries by following standard work for current projects

  • Work closely with other care team members, quality team members to identify areas of improvement

  • Manage patient PCP’s panel of patients utilizing registry reports

Licensure/Certification Qualifications:

Required Qualifications:•    1-2 years’ experience in a medical office setting •    Strong verbal and written communication skills •    Excellent customer service and telephone skills •    Ability to work independently following standard work •    Background with using medical terminology  

Preferred Qualifications:

  • Strong Electronic Health Record (EHR) EPIC skills

  • Ambulatory setting – Primary Care experience preferred

  • Current licensure as a Licensed Practical Nurse in appropriate state, i.e. Minnesota, Wisconsin, OR current registration or certification as a Medical Assistant OR Verifiable completion of a Medical Assistant Program and must become certified within one year of date of hire.  (Must pass certification within two attempts)
