The Community Health Nurse, also known as the TB Case Manager, provides comprehensive community and clinic health services directed towards the prevention of disease, the promotion of health, and the clients self care. In this role you will use professional judgment in comprehensive assessments, screenings, nursing diagnosis, case management, while providing health information and education.
An Ad Hoc Telework schedule may be possible after the successful completion of on-boarding.
As the Community Health Nurse/ TB Case Manager, you will:
* Manage a roster of suspect and/or confirmed active tuberculosis cases that are assigned on a rotating basis with other TB case managers.
* Provide initial assessment and medical history, and develop a comprehensive plan of care for people with tuberculosis disease.
* Work collaboratively with the Multnomah County Health Officer, infectious disease and other community providers to assure appropriate and timely treatment through either direct service or coordination of services.
* Assess clients' needs for in-person and telephone response for urgent and non-urgent medical information; provide TB education using appropriate language and education levels and promote client autonomy and self management.
* Assist clients obtaining other medical or social services or nutritional support as indicated.
* Conduct TB contact investigations (CI's) for pulmonary TB cases within 72 hours of initial contact with the case, including interviewing cases; assessing TB case potential for transmission, and visiting sites where possible TB exposure occurred for environmental assessment of potential transmission.
* For people identified as contacts, provide TB skin tests or IGRA, TB symptom review, and facilitate a chest x-ray(s); conduct TB evaluation including medical history and risk assessment; and conduct second round testing on contacts as indicated.
* Evaluate clients for eligibility for initiation of preventive treatment for latent TB infection; provide education on preventive treatment, and initiate clients on appropriate TB medication regimen following standing orders, nursing assessment and established nursing procedures.
* Maintain clinical skills as dictated by the client population and the clinical practice; review cases with the providers and team as needed.
* Provide phone advice and consultation to Multnomah County health care providers and the public on Tuberculosis and communicable disease issues on a rotating assignment with other members of the CDS team.
* Complete all state and federal reporting requirements according to guidelines and policies; and use appropriate electronic systems for documenting and tracking medical (e.g., EPIC) and public health (e.g. ORPHEUS) information.
* Mentor, coach and offer clinical expertise to team members; delegate the care of individuals and groups of clients to members of the team based on the scope of practice and clinical skills needed by the clients.
* Support the quality improvement process through team, role, and designated specialty workgroups.
* Respond to public health emergencies and participate in exercises as a trained member of the Health Department Incident Management Team.

Minimum Qualifications/Transferable Skills*:
* A license to practice as a registered nurse as outlined by Oregon Revised Statutes (ORS) 678.021).
* A current BLS CPR certification.
* A valid Driver's License.
* Must pass a reference check.

* Tuberculosis case management experience
* Bachelors of Science in Nursing
* Demonstrated ability to professionally and effectively communicate orally and in writing with a variety of people, answering questions and explaining technical information

* Experience working with culturally, socially, and racially diverse populations
* Transferable skills: Your transferable skills are any skills you have gained through education, work experience (including the military) or life experiences that are relevant for this position.
